You may need to sign a policy agreement upon your arrival for the first appointment.


**Policy Notification**
Thank you for choosing Pure Form Arts for your healing and fertility needs. To ensure the best service for my clients, I have established the following policies.

**No Refund Policy for Pure Form Healing Arts Services and Packages**

At Pure Form Healing Arts, we are dedicated to delivering outstanding services and helping our clients achieve their wellness goals. To uphold the integrity of our services, we enforce a strict no refunds policy on all services and packages. Please review the details below:

**No Refunds:**
Once a package is purchased, refunds will not be issued for any reason, including personal circumstances or dissatisfaction with services. -
**Package Transferability:**
While refunds are not available, packages can be transferred to another individual with prior written consent from Pure Form Healing Arts. The recipient must agree to terms and conditions. -
**Expiration of Packages:**
All packages expire one year from the purchase date. Clients are responsible for using the services within this timeframe. Extensions may be granted under circumstances at the discretion of Pure Form Healing Arts. 
**Cancellation and Rescheduling:**
Clients can reschedule appointments to our cancellation policy. Failure to provide the required notice may result in the forfeiture of the session. 
**Exceptional Circumstances:**
In rare cases, such as severe medical conditions or emergencies, exceptions to this policy may be considered on a case-by-case basis with appropriate documentation. By purchasing a package, clients acknowledge and agree to this no refunds policy and are encouraged to consider their commitment before making a purchase.
**Cancellation Policy**
Please provide a 24-hour notice for any schedule changes or cancellation requests. Cancellations without this notice may incur a $50 charge for the first missed session and 100% of the service fee for subsequent sessions. I may also require a credit card number to secure future appointments for potential charges due to late cancellations. I understand emergencies and illnesses can happen. If you have a fever, known infection, or have experienced vomiting or diarrhea within 24 hours of your appointment, please cancel your session. Late cancellations due to emergencies or illness are generally exempt from charges but will be evaluated case-by-case.

**Late Arrival**
I understand that delays can occur. Please inform me if you're late so I can do my best to accommodate you. However, arriving late may result in a shortened session, and full service fees will apply. I will strive to be on time and will add time to your session adjust the service charge if I am late. For questions or concerns regarding this policy, please contact us at (559) 270-3310.
